Dr. Forges Madistin, DNP, APRN, FNP-C, is a highly motivated, energetic, and compassionate Advanced Practice Registered Nurse with extensive experience in comprehensive patient care, including primary care, STD management, and OB/GYN services. As a Doctor of Nursing Practice, she brings strong clinical and health assessment skills that enable her to accurately diagnose conditions, implement effective interventions, and deliver high-quality, patient-centered care within a multidisciplinary healthcare team.
Dr. Forges Madistin is deeply committed to treating every patient with kindness, empathy, and respect. She is well-regarded by colleagues, friends, and family for her unwavering dedication to improving the health and well-being of others. A strong leader and mentor, she actively supports and encourages staff to perform at their highest potential, fostering a collaborative and empowering work environment.
Originally from Haiti, Dr. Forges Madistin began her healthcare journey as a dentist decades ago. After relocating to the United States, she transitioned into nursing, a path that would allow her to remain at the heart of patient care. With determination and resilience, she earned her nursing license and advanced through the field, ultimately achieving her Doctorate in Nursing Practice.
Today, Dr. Forges Madistin is recognized as a valued healthcare professional and a vital asset to her community and the state of Florida. Her dedication, integrity, and passion for health care continue to inspire those around her and positively impact the lives of countless patients.
Dr. Mario Theliard is a highly qualified and compassionate family physician with over 25 years of experience in the practice of medicine. He earned his medical degree from the State University of Haiti School of Medicine, where he also completed his Internal Medicine residency at the university’s hospital.
After successfully passing the United States Medical Licensing Examination (USMLE) and meeting the requirements of the Federation of State Medical Boards, Dr. Theliard pursued further specialization in Family Medicine in Puerto Rico. His commitment to excellence in patient care led him to advanced training in HIV management at both New York Presbyterian Hospital in Manhattan and Jackson Memorial Hospital in Miami.
Dr. Theliard brings a wealth of clinical expertise to the community, with a strong focus on family medicine, HIV, and STD screening, treatment, and prevention. His approach is deeply rooted in accessibility and equity—he is fluent in English, French, Creole, and Spanish, allowing him to bridge language gaps and reduce health disparities across diverse populations.
Dr. Theliard is not only a skilled clinician but also a dedicated advocate for community health, known for his empathetic care and culturally sensitive practice.

Sexually Transmitted Diseases.
Sexually transmitted diseases (STDs) are a major public health concern, and they can have serious health consequences if left untreated. The Centers for Disease Control and Prevention (CDC) recommends that people in certain high-risk groups—such as people who have multiple sexual partners, people who engage in unprotected sex, and people who inject drugs—get tested for STDs regularly.
Immigration Physical.
An immigration physical is a medical exam performed by a physician approved by the U.S. Citizenship and Immigration Services (USCIS). The exam is used to determine if an immigrant meets the medical requirements to be admitted into the United States. During the exam, the physician will check the applicant’s health condition and take a medical history, including vaccinations and past illnesses. The applicant may also need to provide additional tests, such as chest x-rays and blood tests.
